Our experienced psychiatric nurses are dedicated to helping patients coping with acute, chronic or debilitating illnesses and for patients with primary psychiatric diagnoses such as depression or anxiety. Our Behavioral Health Program offers an interdisciplinary approach to create continuity in care for patients. VNA/SCC’s comprehensive care helps patients achieve the maximum advantages of home care. We track and manage all details of each patient’s care and administer medication at prescribed times. We also provide feedback to the physician as well as instruction to the patient and family. Our ongoing assessment of mental status and ability to function provides critically important information to primary care physicians. And our access to community resources makes complete patient care possible.
VNA/SCC professionals work in close collaboration with physicians to develop and implement the most effective personalized care plan for each patient.
- Ongoing assessment
- Support and training for patient and family
- Evaluation of home environment and support systems
- Interdisciplinary conferences with physicians and health care team members
- Administration of injectable neuroleptics
- Referral to community resources
- Coordination of ancillary programs
- Medication administration 1-2 times/day, if needed
